why? those patients had a choice and they accepted the treatment he offered. With his years of experience, can you guarantee the patient will maintain the thousand of dollars restorations and root canals on those teeth when their prognosis is questionable at best when they could not adequately take care of their teeth? Sometimes there is so much damage that replacement with implant and dentures is not a bad option for the patient. Are you even a dentist?

Always thought about adding glue to the screw but don't you think that screw loosening may be good as a red flag that tells when something is going wrong force wise, otherwise, this detrimental force are going straight into the implant and bone.

Has this done and a wisdom tooth filling 8 hours ago, no anesthetic was used for wither of the procedures. A little bit of discomfort from the srp and even the filling wasn't that bad other than a couple times the drill must have hit nerves which caused me to shiver lol. The only way I can describe the pain is like a screeching when you rub nails down a chalk board
i know this video is 3 years old, but i'm hoping you'll see this. are you sealing off the gutta percha with anything? if so, are you just sealing the gutta percha or are you sealing off the dentin as well...i.e., mesial to distal fuji/composite? are you grafting on top of the root at all? or just rely on the blood and temporary pontic to mat up the blood to heal?

Doc do you do cantilever bridges often? If so, what situations?
I do not cantilever fixed bridges often, simply because I must depend on the patient not to bite anything hard on the Pontic or the bridge can debond or the abutment tooth can break. I want things I restore to last, and cantilevered bridges do not have predictable long term success. I will sometimes cantilever a maxillary lateral incisor off a cuspid, especially if the patient is not a clincher and has a nightguard. You do not want to ever cantilever the Pontic distally off a tooth that is medial to the Pontic. The stress on the Pontic increases as you move distally in the mouth. Bottom line, you do not want the patient's problem to become your problem by restoring something that is not going to hold up. You do not want to get into a situation of having to decide who absorbs the cost of redoing or recementing the restoration if comes off or something breaks. Do not create problems for yourself. Remember, "emotional energy has a dollar value." Take your practice to the Top Tier. Possibly. It may be a "transient paresthesia," which means the inferior alveolar nerve was not damaged, just shocked from the pressure of the lower wisdom tooth being extracted. If it is a "transient paresthesia," the feeling in your lip should return within 6 months. I would have the surgeon examine you and advise.

do you ever separate some of the pdl first before using the leverage system? I tried this system once but the tooth split in half (it was previously treated for RCT)
The possibility of a tooth fracturing using the EasyXtract system exists. Still, it is a valuable tool if the tooth is broken off or decayed at the gumline and there is no "purchase point" for extraction forceps and you need to preserve the facial and palatal/lingual bone for an implant. Luckily no dilaceration otherwise this will not work.

If a tooth has dilacerated roots then path of removal gets obstructed. But this method requires an unobstructed and straight path of removal.
This method is for extraction of single rooted teeth. It can also be used for extraction of a single root of a multirooted tooth if the root is wide enough to accommodate the drill. It is not for extraction of all teeth, but is invaluable if the tooth is fractured off at the gumline and you want to preserve the facial and palatal alveolar bone for implant placement. Hello doc , its been almost since i started practicing dentistry i have watched most of your videos on youtube and it helped me to get better at dentistry. I would like to ask your help with one specific thing how to easily see lower first molars mesial root tips when they fracture, i really have a hard time getting those lower molar mesial root tips out especially in the third quadrant even the distal root tips are kinda not so hard for me but these mesial root tips cant see em.. .do periostal flap and little bit of buccal bone removal always necessary to see them .

Sir - why did you wipe away the excess cement for this inlay? In other videos where you cement crowns, you teach us to scale off the excess when there is initial set of the Resin cement.
Very good attention to details! I am so happy you noticed that. The only time I remove excess cement prior to initial set of the cement is with gold inlays. The reason I remove the cement prior to initial set with gold inlays is the margins fit so well that there is minimum micro gap between the inlay margin and the crown and the margins are not under the gingival tissue, so I am not concerned with gingival irritation from bacterial accumulation subgingivally. Plus I polish the margins of the inlays after seating. After placement of gold inlays for over 40 years,I cannot think of any gold inlay margin ever leaking. The seal is incredible if the impressions are accurate and the technician is excellent. Very good attention to the fine points I am teaching. I am glad you caught that point! @@centerforard Thank you Sir! I have studied a lot of your videos and have membership of DMC with a colleague of mine! We have both agreed that it would be nice to see an onlay prep video too. We love a gold onlay - the trend seems to be building up a broken cusp with amalgam/composite but what's wrong with providing cuspal coverage with a gold onlay? I wish gold was used more consistently amongst dentists... If you could perhaps expand on your philosophy when restoring broken down/decayed cusps on teeth with an onlay and demonstrate expert techniques in a video showing this, we would enjoy such a video. I will say practicing comprehensive dentistry for over 40 years has been very good to me and my family. I am shocked by the lies still being perpetrated by the dental industry. I just googled the subject of danger of root canal, written on Colgate website. It says: Despite the claims you might read on the internet, root canal treatment doesn't cause illness. The American Association of Endodontists (AAE) reassures patients that there isn't any scientific evidence linking root canal treatments to illnesses or diseases elsewhere in the body.Jan 9, 2023
What is your point. An endodontic procedure is a procedure that can be performed to save a tooth vs. extracting the tooth. The only options for an abscessed tooth (dead nerve) or a hypersensitive tooth are endodontics (remove the affected nerve) or extraction of the tooth. If a tooth is responding negatively to any procedure, especially a root canal, there will be a widened periodontal ligament around the roots of the tooth which can be clearly identified on an x-ray. If the affected canals of a tooth are filed and irrigated thoroughly, these teeth can last many, many years. There are no scientific studies that associate root canals with systemic illnesses or diseases. I personally have had about 4 root canals from sporting accidents and I am a very healthy person with no known systemic problems, and those teeth function perfectly. I would prefer to have a properly performed endodontic procedure than a tooth extraction and an implant or fixed or removable bridge if given the choice. It is certainly not a financial gain for the dentist to perform the root canal. An extraction and tooth replacement with an implant or fixed bridge is considerably more expensive than a root canal. You can always find some group that claim some medical/dental procedure is bad for you. I won't even go down that road. If some group just has a personal opinion about a procedure being bad for you, without studies to back that opinion, then what can be said. In dentistry we probably would be limited to only extraction of teeth

If I am restoring 8-10 adjacent teeth with veneers, I usually lighten the shade, normally to a B-1 or A-1 vita shade or even lighter A-1 or B-1+ if it is for a woman who wants considerably lighter shade smile. I refer to the shade of veneer cases in the Aesthetic Restorative Dentistry picture book with my technician., plus I have worked with the same technician for many years, so he knows the shade I am wanting when I request for instance, a B-1+ shade.
